Apartment Units for Homeless Individuals and Families with Mental Illness Eligibility Criteria: - Acceptance of male and female individuals and/or families aged 18 years or older - Diagnosis of severe and persistent psychiatric disability - Dual-diagnosis of concurrent substance abuse disorder (if applicable) - Homelessness status - Commitment to remain clean and sober Program Requirements: 1) Community Engagement: - Participation in volunteer work, employment, or pursuit of educational opportunities - Motivation to interact and contribute to the community 2) Medical and Behavioral Health: - Attendance of all medical and behavioral health appointments - Self-administration of prescribed medication 3) Program Fee: - Monthly payment based on income Providing Supportive Services: - Stabilization and preparation for independent living - 25 apartment units offering 1 and 2 bedroom options - Supportive services tailored to meet the needs of homeless individuals and families with severe and persistent mental illness Funding Source: - U.S. Department of Housing & Urban Development (HUD)
