Original documents for court-ordered separations, child support, divorce, legal custody, etc. 4. The most recent six-weeks of pay stubs and employer contact information. 5. Copy of Section 8 voucher (if applicable). 6. Verification from LG&E for new service eligibility (confirmation number or account details).
